Before coming in for your first visit, please complete our new patient paperwork. It helps us to have some background information so that we can hit the ground running! If you have any issues filling out the forms, please call/text us at (419) 994-2424. We will do our best to assist you before your visit or offer you a printed version to fill out in the office.
